What is the Community Care Program?
The Community Care Program allows eligible veterans to receive health care from local providers, not just from VA facilities. This means you can come to Cleveland Chiropractic and Massage for treatment with Dr. Moran, who specializes in helping patients manage pain and improve their overall wellness.
Steps to Access Care
Here’s an easy-to-follow overview of how you can access chiropractic care through the Community Care Program:
1. Visit Your VA Doctor
Start by scheduling an appointment with your VA doctor. Let them know of your pain and that you would like to seek chiropractic care specifically with Dr. Moran.
2. Consult Review
Once your VA doctor submits the consult request, the Community Care department will review it to make sure everything is accurate. They want to ensure that you get the right care!
3. Scheduling Appointments
After your consult is approved, the Community Care department will contact you and Cleveland Chiropractic and Massage to set up your appointment. If you prefer to call and schedule, just remember to confirm with the VA that you have authorization.
4. Authorization Letter
You’ll receive an authorization letter in the mail that includes important information, such as your authorization number and details about your chiropractic care. This letter is your passport to treatment.
5. Your Appointment with Dr. Moran
On the day of your appointment, you’ll meet Dr. Moran, who will assess your condition and create a personalized treatment plan to address your pain. She and her team are especially dedicated to helping veterans regain mobility and comfort.
6. Ongoing Care
After your initial visit you’ll be able to book follow-up visits directly with Dr. Moran, as long as your care is approved. If you need to reauthorize future referrals, Dr. Moran’s expert team will help guide you through this process.
7. Reauthorization
If you need continued treatment past your initial authorization, your VA doctor will need to submit a new referral request to get you reauthorized for care.
